    FMLA/Florida/Surgery

    What is the name of your state (only U.S. law)? Florida

    I have been having memory loss, originally believed to be from high blood pressure, but then my dr believed it could be related to limited oxygen flow b/c of my severe deviated septum causing sleeping/breathing problems, made asthma worse etc...

    I am under FMLA for highbloodpressure/memory loss and then surgery was suggested for my deviated septum. I applied for FMLA paperwork for my deviated septum and the surgeon/ENT is telling me that he filled out the paperwork for my surgery and sent it to the clinics health information department, but the health information department is telling me they don't have it and are waiting on the doctor.

    In the case that my dr and health information desk cannot straighten this issue out when it's due, am I still covered because the surgery was in hopes to correct the memory loss problems? I have also been diagnosed as ICD-9 780.93 for retrograde amnesia/Memory Loss "NOS" by a neurologist who does testing for the FAA and military.

    If your surgery falls within the time period for which you are currently approved, you will be covered. If your surgery is not within that time period, you must apply for an extended period of time. If the paperwork is "lost", you must submit a new form from your surgeon.
