Health history waivers

What is the name of your state (only U.S. law)? Texas
Can you have employees sign waivers regarding disabilities or health issues? For example, if an employee is hired to remove debris and he has a previous back injury, then he later claims the injury happened at work and it becomes a work comp issue. We are trying to avoid situations like this. Can employees be forced to disclose health history and sign waivers of prexisting conditions so that they will not blame the company later???What is the name of your state (only U.S. law)?
