Never sent COBRA info

What is the name of your state (only U.S. law)? CA
I was injured on the job and threatened to not go on Workmans comp or I would be fired. I was eventually terminated anyhow and then filed the claim I thought it was an open and shut case (thats what the lawyer told me) so it never dawned on me that never recieved any COBRA info from the employer.

I thought that my workmans comp filing would pay for the surgery I need. Now suddenly the Workmans comp has been denied and I am thinking that I could pay for the COBRA and get the surgery while everyone fights it out.

What are my options, I never received the COBRA notice, they do have more than 20 employees. What do I do. I need this surgery on my leg in order to get a new job.

Injury happened Sept 8th 2008. They forced me to keep working and not report it using threats, until they fired half of the staff April of this year.

I am suing but they have rejected my workmans comp claim... I still want to keep the lawsuit going but I can't get a new job until I get my leg fixed and I am dying on the vine here on disability. I need to figure out what my rights and choices are in getting the surgery done, while I wait for this long stupid process to be played out

Did your employer or did you fill out an accident report on the injury? If so, did you get a copy?
