What is the name of your state? What is the name of your state? OHIO :p

I INJURED MY BACK IN JUNE OF THIS YEAR AT MY PLACE OF
WORK. I HAD TO HELP A MAN THAT SHOWED UP FROM THE MAIN
OFFICE CARRY A CONVEYOR BELT DOWN FROM UPSTAIRS TO HIS
TRUCK. THIS CONVEYOR BELT WAS ABOUT 15 FEET LONG AND AT
LEAST 300 POUNDS. I TOLD MY SUPERVISOR ABOUT THIS WHEN HE
RETURNED TO WORK AND HE SAID HE WOULD MAKE UP A REPORT.
I'M NOT SURE IF HE EVER DID. I RECENTLY QUIT THIS JOB BUT
I'M HAVING SOME REAL PROBLEMS THAT I THINK ARE RELATED TO
MY BACK INJURY. I HAVEN'T BEEN TO A DOCTOR BECAUSE I CANT
AFFORD IT BUT NOW PARTS OF MY FEET AND FINGERS ARE GOING
NUMB. CAN I STILL TAKE THIS TO A WORKERS COMP CLAIM EVEN
THOUGH IT WAS MONTHS AGO AND I NO LONGER WORK THERE? THANK
YOU...

No one can predict what the outcome will be but you need to contact this former employer and tell them you are having back problems that you believe are related to the incident in June and will be seeking medical treatment. The employer will file an injury report with their WC carrier (which they almost certainly did not do back in June since you never saw a doctor) and then the WC carrier will make a determination regarding liability after they receive the appropriate and necessary information from whatever doctor is treating you.
