What is the name of your state (only U.S. law)? Oklahoma

Hey everyone,

Ran into a problem.

Recieved our CO in the mail Saturday. Everything is correct. However, one (possibly?) major detail is missing. Our son's name change is mentioned nowhere in the order. We had both agreed to hyphenate his last name.
It was overlooked by all of us (Both attorneys and Dad and I) before we signed the order.

How do I fix this?

ETA: Dad's attorney wrote the order.

Who wanted the name change? You or dad? If it was you, have your attorney put paperwork together to deal with the omission.
