Can an HOA place a lien on your house without notifying you besides giving an intent to lien letter?

They did notify you. That's what an intent to lien letter is...notification that they intend to place a lien.

There was also notification in the CC&Rs that your home was subject to lien if you didn't pay, plus I'm guessing that you got plenty of late notices.

My guess is that they could have filed the lien the first time you didn't pay and the lien would have been your notice of the lien.
