1099 is not provided by hiring company.

I was contracting for the company ( start up dot com)
for three months in 2000.
I thought that that company supposed to provide me with 1099 form indicating how much i recieved from them.
I want to make sure that our numbers do match.
I tried to contact them but in vein. anything that i can do to get that form .
is it the law that they supposed to provide 1099 and if it is, what penalties they may recieve ?
