What is the name of your state (only U.S. law)? Michigan

I received a gross distribution as a beneficiary to my Mom's annuity w/ING USA. No taxes were taken out, total was just under $3,200. Do I include this whole amount on my taxes this year as income? Money received in 2008. Thank you for any help.

It would go on your 2008 tax return. See instructions for 1040 line 16, or 1040A line 12. If you already filed you should do 1040X before April 15.
