2 accidents-insurance companies not paying!

IDAHO - dec 9th, 2000 my husband hit another car that had run through a stop sign. resulted in lower back injuries and shoulder pain. family doc prescribed meds, physical therapy, and eventually a visit with ortho-surgeon. on april 4, 2001 (4 months later) he was rear-ended at a stop sign. this resulted in a neck injury and shoulder pain again. 1st accident, MRI showing a disc in lower back missing. 2nd accident, xray showing a bone fragment at base of neck. these both were ordered after apr 4th. due to a billing error, the two insurance agencies have become aware of the other accident. 1st insurance co refusing to pay med bills after apr 4th (the mri) the 2nd is refusing to pay because injuries could have been sustained from first accident. both have offered $1000 for pain/suffering with no responsiblity toward med bills. husband still having back problems, so we are not ready to settle. we have been very careful about telling doc which injury is from which accident. husband has never had back/neck/shoulder problems before now. doc and ortho-surgeon are neighbors and know he's had good health until now. questions:
-can I solve this without an attorney?
-$1000 seems like chickenfeed for missing disc and bone fragment (lifelong injuries) docs say he will always have some pain. are these offers standard for these type's of accidents?
-if we pay med bills out of our own pocket will it be harder to recover from insurance co's?
HELP!
