What is the name of your state (only U.S. law)?Florida

I need a three day notice for a hold over tenant. Demanding double the rent or vacating the premises. I have searched high and low for one but can't find one though it says it exists. Can anyone help me with this since I want to make sure it includes everything needed

did the tenant give you proper notice to vacate or did you give proper notice to vacate ? either which way if proper notice was given then WHY are you seeking information as to notices to quit ? Normally when proper notice is given and tenant still wont leave then its time to go file for a eviction complaint based on tenant over staying the landlord or tenants own notice to vacate. You wrote >Demanding double the rent or vacating the premises.< what makes you think a court is going to allow doubling the rent ? You either want them out or you do not.
