401k amortization schedule and paycheck payments were different days defaulting loan

Help, amortization schedule and paycheck payments were different days defaulting my 401k loan.

I set up loan thru fidelity and my company.Loan payments were coming out of my check but the dates paid to Fidelity was off. June 28 was my amortization schedule due date but the company paid July 2 which defaulted my loan.
No one is taking responsiblity. Why were the loan payments off???
