What exactly must be proven to a judge before he/she can determine whether or not a mother is a "dangerous threat" to her children? Are we talking about a danger that the children will be raised in such a way that will negatively impact their adulthood or are we talking about a potential for "bodily harm"?

Proven physical harm. Generally.

No one is going to helpfully list all the ways you can "prove" a parent unfit. That's irresponsible: imagine what readers could do with the info.

More often than not, Judges go to "experts " on these things..which means he/she would have you, the mom and the child (if old enough) to go to a psychologist appointed by the court to determine is there are any issues. This will cost you a pretty penny.
