What is the name of your state (only U.S. law)? NC
Is a specific form required for my son and daughter in law to name me as legal guardian in the case of death of both of them . . . or temporary guardian if my son is deployed and my daughter in law dies? Or is it just an added statement in their will?

No such thing. They can set up a standby guardianship but they would need the help of an attorney. That is provided that your son and daughter-in-law are the legal parents of the child.
