Affidavit of Parentage

What is the name of your state? I live in Michigan....Do both parents have to sign the affidavit of parentage for it to be official or can one parent process it by themself? Both parents are unmarried in this case....

Both must sign in front of a notary. It's in the instructions on page 2.

ETA: The form also allows for a "qualified witness" instead of a notary. Qualified witnesses include hospital employees, etc. Also, the form doesn't have to be signed by both parents at the same time - they can each sign separately, but, when the form is submitted, it must have both signatures properly notarized/witnessed.
