if an insurance company has denied a claim on life benefits and i (the beneficiary) file a voluntary appeal and they tell me they have up to 45 days to review the claim, what happens if they havent responded and the 45 days are up

Are you saying the ins. co. is reviewing the claim/appeal? Are you sure you are the beneficiary? If so, why did they deny the claim originally? I assume you are talking about life ins. benefits.
