The first service provider should be responsible but the primary target is your own insurance company. Contact your State insurance commission and get their opinion on the matter. It would appear that the regulations to protect the consumer were not followed. You need to be more forceful with your insurance company. Send them a certified letter giving them a time limit to get your van fixed and ready for you to drive it home. If they will not accomplish this, demand that they deliver to you a brand new fully insured Mercedes SUV. Let them pick the color though so they will not get too upset.
