Being sued by hospital, but making payments...

A friend of mine had an accident in 1996 and has religiously paid $50 a month to the healthcare facility. On numerous occasions they have had her sign paperwork that involved her obligation of repayment. Now they say $50 is not enough, and are pursuing this in court. They healthcare facility stated that they would continue to accept the $50 payments, but want her to sign more legal documents stating that she is still obligated to pay and she now has a 10 day grace period to pay each month or they will sue her for the complete amount. If she has been making these payments for 4 years with no real threats from the facility about the amount she is paying, can they come back now after four years and pursue a higher amount of payment? Could she possibly be being set up to have her wages garnished? If so, would that be legal being that she has faithfully made payments every month?
