What is the name of your state? Oklahoma. My questions is a friend named my on a life ins policy as a beneficiary but we forgot to send the changes in. He recently died is the form still valid . we filled it out in june of 2002

I believe that the change of beneficiary must be received by the insurance company to be effective.
