What is the name of your state (only U.S. law)? SC

I am trying to cancel membership to Massage Envy, the contract has ended but they say we have to fill out a 'cancellation form' for a 'proper' cancellation. They also put us on autobill and charged us for this month, then said that they do not offer refunds! Can I get a refund from them legally? Will filing a dispute with my bank help? The form they want us to sign is basically an arbitration agreement, do we have to sign that or can we write our own cancellation letter/email and cancel that way?
Thanks for the help!

What does your contract say about cancelling membership? Is there an automatic renewal of membership if you do not notify Massage Envy in advance that you do not intend to renew?

It is the contract that you entered into with Massage Envy that will govern what you can do.
