If the debtor has changes in income prior to confirmation in a Ch. 13, are they obligated to report those to the trustee or amend their filing or something? For all we know, this fool has doubled his income in the last year since filing, and there's still no confirmation. So are they going to determine the percentage based on that old info?

And if you have an adversary proceeding within a bankruptcy, the debtor's attorney obviously is going to get paid dollar for dollar out of the estate, and before the other creditors. So he doesn't really have any reason to NOT want to be contentious and prolong the thing with various different maneuvers that essentially delay a resolution, right? Or is that too cynical of me?

The debtor's attorney should have amended his schedules and his chapter 13 plan if his pay has increased or decreased.
