collection agency or original creditor???

What is the name of your state? ny. when filling out the list of creditors do I put the original debtor's address or do i use the collection agency's address and info? Also my husband signed an agreement about 6 months ago to pay medicaid back for the medical expenses incurred during my pregnancy (we weren't married at the time of her birth) is it possible to put this on his bankruptcy or do we just leave it off there and keep to the agreement? thank you.

You have to list ALL debts, including the Medicaid pay-back. Don't know if that'll be dischargble since its a Federal program, but you have to list it. You can always pay it back later if it does get discharged.

List ALL creditors, collection agencies, etc. Anyone and everyone you owe money to you list. List the original creditor and under each one of those list the collection agency(s) for that same account. You don't want to leave off a single one. If you've got your credit reports that's probably the first best source to use.

thank you. so i also need to put down current debts that we are currently paying on? (we have credit thru a local furniture store and we are current on the payments and don't want to lose the credit if possible)

If its a debt, you have to list it, period. Even if you 'accidentally' left one off, there's no guarantee it wouldn't be cancelled anyway once the BK hits your credit report.
