confused about custody laws

What is the name of your state? Arizona
I have a thirteen year old daughter, her mother has had nothing to do with her since she was three months old. Her mother and I never married and when she was born my name was not put on the birth certificate at that time. With in the first year of my daghters life I had my name put on her birth certificate at that time we both signed an affidavit of acknowledgment of paternity. Although I have raised my daughter for the past thirteen years without having sole custody or any custody apponinted by the courts, I need it now. My father has just passed away and I would like to relocate my family (my daughter and my self) to Ohio. The mother of my daghter has had numerous problems with the law. she has served a prison sentece, shes addicted to pain medication, and also has been arrested for prostitution. The mother has just heard about our pending relocation and is making accusations that I'm not the father and that she is going to take her away from me. To be honest I don't know if I am really her father and I don't care to know. I am the only father or parental figure this child has had. I can't afford a lawyer and I don't know how to start a custody case. I would like to have her parental rights terminated. If there is any advice you could give me to lead me in the right direction, your time would be greatly appriciated.

Confused about custody laws.
If you have been the legal father for 13 years no judge is going to take that away from you, so please don't worry about that. If mom honestly has had no meaningful contact for 13 years it would also be tough for her to stop you from moving.

However...at a minimum you need to get a consult with an attorney...and it would probably be best for you to plan to find a way to retain one. You definitely need to get your legal custody established.
