What is the name of your state (GA)?
What's form for "Consent to Agreement"?
I found one which seems to close to is "Waiver of Jurisdiction -Respondent's Affidavit". My wife may not want to go the 31-days after service trial.
Is this form right form?

I have been informed that I have a constitutional right to a trial by judge or jury in the state of myresidence, and that I expressly waive my right to jurisdiction in the state of my residence, and consent to jurisdiction in the State of Georgia, Fulton County Superior Court.

Why don't you start by telling us what you want to do and what the circumstances are?
I try:

Both my wife and I want to avoid going the county court for the divorce hearing if possible. We have had the agreements on all the items of the Separation Agreement provided by the family court of the county.
Question: What do we need to file for avoiding of the court hearing?

The only way I have ever heard of the parties avoiding going to a court hearing is if they are represented by attorneys who submit the agreements/paperwork to the court.
