converted utma cost basis

What is the name of your state? Florida

My UTMA account was recently converted to an individual account. The cost basis is currently unknown. Do I have to go back and find the original cost basis, or do I get a new cost basis since the account was converted?

Whatever investments were held in the account (stocks, bonds, mutual funds, etc) retain their cost basis. Changing the type of account doesn't change that. So, yes, you need to do some research.
