creating a terms and conditions for a fan club

What is the name of your state? UK, but the club is open to everyone in the world.

i need to find a template or find a fan club that has t&c's for joining the club, i want to create a fan club for an actress and need them to start it.

does anyone know where on the net i can get one from. or does anyone have membership with a fan club can you let me know who it is with and how i can join who to phone up.
