What is the name of your state? Ohio

I am filling out paperwork to be seen at the dentist tomorrow. It asks how you will be paying for your copay and you mark cash, check, or credit/ATM card. If you mark credit/ATM card they then request you put your card number and the expiration date on the paperwork. This makes my card numbers part of my permanent records. I am not comfortable with this. Can they legally make you put that info in the file?

They can't legally MAKE you do anything. You are within your rights to refuse to fill that information out and state that it puts you at risk for identity theft and you do not want that information "on file". You also don't have to give them your SSN, another ID theft risk. They won't like it if you refuse, but they can assign you some other identifying number.
