Is there any form that can be used for a family member (other than a parent) to sign that is taking a child for an extended visit.

I want the person to be responsible for the child while in her care and custody but I also want insurance that she will return the child at the agreed upon time and date and that if she does not that she will techinically be a kidnapper.

I know it sounds confusing and that's the reason I'm here asking !

I live in Florida and the child lives with me the trip and visit would be to Pennsylvania.

My response:

Go to your nearest computer store and pick up a computer program called Quicken "Lawyer 2002" - - Personal Edition.

After loading on your computer, you'll find easy-to-fill-out forms, two of which are:

1. Child Care Agreement, and

2 Medical Authorization for Minor.

The program costs between $40.00 and $50.00, and will do the trick.
