I met with my lawyer the other day and I have to fill out this form and bring it back to her tomorrow. It is asking what kind of outcome I would like to see in the custody hearing.

What exactly is joint legal and physical custody? What I would like is for us both to have a say in our childs decisions, but I want to be the primary custodial parent. Is this considered joint physical? He is asking for EOW, alternating holidays, and time in the summer. All of which is ok by me. What is this called?

Thanks

According to what you said, you want full physical custody and joint legal. You want your ex to have reasonable visitation.

Your lawyer should be able help familiarize you with all the terms involved in a custody case.

Yes, but you should spell out exactly what is meant by "reasonable visitation", because what is reasonable to you might not always seem so resonable to him. If you're going for EOW, alternating holidays, etc, be specific. What counts as a holiday? What if your holiday falls on his weekend? Who does the driving? Lots can happen...
