1. The State this incident occurred in;
2. Your daughter's age;
3. The facts of how this incident occurred;
4. Where the dog was at the time of the incident.
5. A description of your daughter's injuries - e.g., puncture wound, tear wound, and how many bites, including location on her body.
6. Any other relevant information.
