For the past 6 months Blue Cross of California's Group Medical Insurance underwriter has been making us jump through hoops to qualify for the group health medical insurance HMO. We have met every requirement requested of us and now they are threatning to deny us coverage without just cause or reason. What can, or what should, I do?
