What is the name of your state? az ... recvd notice from dental office requesting payment from a prodecure that was completed 21 months ago...have never been billed until now...called insurance co, they advised they have been requesting info from dental office, and not receiving correct billing codes, thus the delay in payment...is there a time limit of which they must submit this claim, be paid what the insurance co agrees to pay, and then demands difference from the insured party? thank you...
