What is the name of your state? PA
My dentist has been filing a claim for 4 years to my
insurance compnay. The insurance company has not
paid, saying they don't have all the information. The dentist
has now hired a collection agency to hound me.

Am I responsible for the unpaid amount? Do I have any
options other than paying?

If this is a legitimate claim for which you received services, yes, the doctor can collect from you.

YOU received the services from the dentist; therefore YOU owe the payment. You and your insurance company can fight it out between you (with assistance from the dentist if info from him is needed) as to who pays what, but the bottom line is that the dentist is entitled to be paid for his services and YOU are the one responsible for seeing he gets paid.
