I recently received a letter from my insurance company stating that three line items on one of my dental work statements are charged at above what they consider to be normal and usual charge amounts. In other words, they are stating that the dentist is charging more than what they consider normal or reasonable. They have told me that they are refusing to pay the differnce between what they consider normal and what the dentist has charged.
Is this legal? Can they arbitrarily decide not to pay if they so desire? What should I do if the dentist then comes looking for me to pay the difference? Am I now liable for that difference?
