Did I forfeit custody when I signed?

I'm from Missouri,20 years ld and the parent of an 11month old little girl. I moved out of my parents house and started renting a house with friends of mine. I left my daughter with my parents until I got on my feet and had a stable home for her. About a month after I moved, my parents called and said they had talked to a lawyer about drawing up papers so they could sign for my daughter to get medical care if she wasn't in my care at the time, being it was illegal for my mother to use my daughter's insurance card when she took her to the doctor. I said it was fine and I would sign as long as I didn't give up my rights to my daughter. I went with my father to his lawyer's office to sign some papers and the lawyer told me I wasn't giving up my parental rights, I was just letting y parents care for my daughter until i had a more stable environment for her. Well when we went to court and when i got the papers afterwards, they stated that I given my parents guardianship over my daughter. Does this mean I no longer have custodial right to her?
