What is the name of your state? Nevada
I was paying an extra long term disability ins. premium monthly from 1995, which would pay me 66 2/3% of my annual salary in case I may need it. I went on medical disability 11/2001. Effctve 1/2002 my employer changed the policy by paying me 60% of my annual salary without me having to pay any premium. Because I was paying for 66 2/3% from 1995 to 12/2001, should I not be getting the remaining 6 2/3% that I paid for 5 years?

Not really. An employer can change or discontinue their benefit plans at any time.
