Divorce in CA??

I was married in June of 91, separated (physically, not thru the courts) in 95. Husband sent divorce papers and I signed them, but due to unforeseen circumstances, I had to move and never got the final paperwork. Was contacted by husband recently, who states that we are still married. Husband was active military and was recieving housing supplemental monies while active duty for being married to support his household (ME), only I didn't see any of it, and scraped by on my own.

Would like to file for divorce now. I live in California, husband lives in Indiana. As far as I know, no real property is involved, and no children.

I know I can file myself with self help, but what forms do I need?? Can I get any of that money he got from the military for housing allowance that was SUPPOSED to help support me??
