Divorce papers and name change

What is the name of your state (only U.S. law)? Utah

I recently had my lawyer make a change to our legal divorce papers to include some changes. She wasn't too wild about the changes but went along with them anyway. She recently got re-married and changed her name right before I brought her the papers. The papers have her old name on them but she signed them anyway. I am starting to worry that I need to have her legal name on them. Do I need to do it again with her legal name?

What papers were these? A modification OR were the divorce papers NOT filed yet due to the changes but you had a hearing and therefore believe you were completely divorced?

We were divorced 7 years ago and had signed a decree for how to deal with our child custody matters. I made a change to this document with my lawyer to update to what is more accurate and had her sign it. It had her old name from when we were divorced. She has now changed her last name since she got remarried.
