What is the name of your state (only U.S. law)? Texas

Situation:
My wife and I divorced in 2000. We parted amicably. Our legal counsel did not inform us we needed to file anything to remove her name from our co-owned home when I bought her out of the property at the time of our divorce. I now am preparing to sell the home (which is owned by me - i.e. no lien-holders). She now lives in Canada, but has no contention with signing the required documents.

Questions:
What form is required to get her name removed from deed/title?
How do we notarize the document with her in Canada and me in the U.S.?
Are there any other potential issues I should be aware of?

Craig the cleanest way likely is to hire a atty in your area to review your buy out of her and see what they advise for removing her from the title even if that means contacting her with documents for her to sign.
