What is the name of your state? What is the name of your state? Iowa.
I injured my back several years ago while employed at a VA as an RN.
I had a neuroligist as my primary physician. Workmans Comp sent me to a back specialist for his opinion. What he wrote on my report was awful! I had told my neurologist that my mother had a history of bipolar disease and had recently been hospitalized. The back specialist wrote that it was a wonder that I could work as an RN at all with my diagnosis of bipolar disease and recent hospitalization. He also wrote that I was familiar with the medical "lingo" and wondered who my drug dealer was! He also wondered if I had sued anyone and stated that I had totally manipulated the system. This doctor was sued by another patient for a messed up surgery and he is no longer in practice. He still lives in the same city. The things that he wrote on my permanent record were inaccurate, pure subjective speculation and totally unprofessional. What is the statute of limitation and is there anything that can be done especially since he is no longer in practice?

You can make complaints to your state medical board and to professional organizations if he belonged to any.
What damages did you incur as a result of these notations?
Have you submitted amendments pointing out the innacuracies?
Do you have a copy of your record, is it possible that he misinterpreted what your neurologist wrote, have you seen what your neurologist wrote.
Consult with an attorney.
