What is the name of your state (only U.S. law)? Virginia

If a husband and wife have a separation agreement, does that agreement still stand if they re-unite? What if the husband and wife again decide to live apart from each other after a period of co-habitation? If none of that matters, how can the existing agreement be reversed if both parties mutually agree that it should be changed?

Yes Co-habitation nullifies a seperation agreement

Yes if you have reconciled and start living together again your seperation period would have to start all over again.
