What is the name of your state? Tx

i recently received the Clean Slate Program Affidavit. i was reading it over and it looks fishy. signing this looks like it only covers members of the RIAA leaving me open for other lawsuits. i feel that if i had signed it other companies could obtain my information identifying as someone who admitted to a crime. i didnt really download but a few songs. but i dont want potential lawsuits falling in my lap. im i thinking in the right direction or is "the real deal"?
