What is the name of your state (only U.S. law)? Texas

I have Power of Attorney for my dad. If he is deemed incompetent by a physician, is there any additional steps I need to take, like go to court, or are these 2 documents sufficent for me to make decisions on his behalf?

Does the Power of Attorney document state that it is durable - that is, it is still effective after the principal's incapacity?
