Employee out of work on disability, does employer have to maintain health insurance

I have a question form the employers standpoint
The day the employee gives their resignation is the employer required to continue with their health insurance benefits?

Example: employee left work to have major surgery (out for 3 months). on 10/31 them come into the office to say they are resigning to take another job. Typically when company employees quit, if they have coverage, coverage remains in effect until the end of the month. The employee's disability would have ending on 11/11, if they had planned to return.

You are required by law to follow your plan document. If your plan document says end of the month, then it's end of the month no matter why they are leaving.
