When signing a listing agreement, then later signing a status change form, does one form supercede the other?

Also, in Michigan, is it legal to have a listing agreement with a blank in the number of days of the contract? In other words, the realtor didn't fill in the number of days. Would that make the contract null-n-void?

EXCLUSE RIGHT TO SELL

On the original contract, the line where the end date of the contract was left blank.

Later, we signed a status change form to extend the listing to 5/1/2001.
