failure to disclose rate increase

Our dentist charges $107 for teeth cleaning of which insurance pays $100 and the remaining $7 is written off by our dentist because of the amount of business we bring in. Our dentist recently sent us a bill for $140. Upon calling and asking why, we were told that they had raised their rates and we were responsible for whatever our insurance didn't cover. We complained that we should have been informed of the rate increase prior to service. We have now received a bill stating that we owe all of the money the office has written off from our accounts for the past two years.

Is all of this legal- failing to disclose rate increases and charging for monies that have already been written off? Thank you for your help.
