What is the name of your state (only U.S. law)? Virginia

I did post this in family law as well but I am wondering is there a penalty for someone putting false information on school documents. For example emergency forms and enrollment forms.

I already know that it is a misdemeanor to provide false information in regards to residency or age. I assume because that could effect funding as far as the number of children etc.

What I am trying to figure out if there is something that would apply to my ex wife putting down her boyfriend as my children's father. My name is no where on the forms.

I do know that it is against school policy for that county to not provide information about the non custodial parent, but I do not know if there is anything that would apply for providing false information as to who the parents are.

There really isn't much that will happen. You should make note of it, in case you will use this in future court hearings. I agree that it's bull to put anyone other than the biological father in the father spot. Are you close enough to the school that you might do pick ups/drop offs? If you are very far from the school, the mom may be using BF so she can have him pick up the kiddos at school. She went about it the wrong way, though. I agree that it's crappy.
