father gave up rights, how do i switch custody of child to my fiance who she knows as

if the biological father agrees to gives up his rights and the mothers fiancee has been helping take care of the child for over 2 years (she is only 3)is it possible to change the childs last name to the mothers fiancee's and can he (the fiancee) share custody with the mother before they are married. the child only knows the fiancee as a father figure. the biological father has never been in the picture. the mother would like to put the child in to school and would like for her name to be changed first. please help!!!!
