first time filing a small claims case for debt owed

Hello all,

I work for a doctors office that has requested I send an account for judgment in a small claims court. Of course, I have no idea what to do...

We can prove that the pt owes the debt: we have a signed financial agreement saying they agree to pay their portion of the bill, documents from insurance stating what the patient owes, and all records of services rendered and payments made to our office.

The total owed is aprox $1000, I have tried to settle this with the patient, the pt's spouse - they flat out refuse to pay for the services. They even requested that we send them to court for it!

I feel a little intimidated... I am confident that we have a strong case, but how do I go about filing with the court? What is "protocol" for Harris county?
