Filling out the form as the payee herself. How do I answer question 3?

The form seems to expect the representative payee to fill out the form. How would I answer the question if I am not the representative payee, but the overpaid person herself?
Also, the representative payee is filling out another ssa-632. We are both requesting a waiver and, therefore, filling out separate forms. Yet the representative is mentally disabled and must ask outside counsel to fill it on his behalf. How can he let social security know that the form was filled out by someone else at his approval? Should a letter specifying this be attached to the completed form?
