former landlord demanding rent after premises was vacated

Over two years ago I moved out of an apartment at the end of my lease agreement, and with the proper notification to the landlord. After reviewing my credit report recently, I discovered that my former landlord has charged me for an additional two months rent. The landlord claims that I did not vacate the premises until two months after I actually did. I had all of the utilities shut off on my move out date, as well as have several witnesses who helped me move. The keys were turned in to overnight deposit box at the rental office on the date that I vacated the premises. Along with a forwarding address and phone number. I also have a witness for that. I had phoned the rental office two days prior to the moveout date. My question is what recourse do I have in order to resolve this dispute? What evidence do I need in order to back up my claim?
